VOLUNTEER CONFIDENTIALITY FORM

DEFINITIONS:

In this document, the following terms have the following meanings:

AGREEMENT - this document, as drawn between Phoenix French Bulldog Rescue and the named individual

RESCUE - the Phoenix French Bulldog Rescue

TEAM MEMBER - any serving member of the Phoenix French Bulldog Rescue Team

MANAGEMENT TEAM MEMBER - any serving member of the Phoenix French Bulldog Rescue Management Team

TRUSTEE - any such person as holds the post of Trustee of Phoenix French Bulldog Rescue at any given time

PROPERTY - any equipment or items provided to me by Phoenix FBR for the purpose of carrying out my role

I understand that, during my time as a Volunteer, information will be disclosed to me that the Rescue considers to be of a confidential nature. I also understand that it is important to the business of the Rescue that such information be kept confidential and that, to this end, the Rescue will require that information not be disclosed or used except as agreed by the Rescue in advance.

I confirm that I agree to keep confidential all information disclosed to me by any Team Members, and all Rescue business, during my time with the Rescue and afterwards, if I leave. I confirm that I will not make use of such information without prior written agreement with the Founder of the Rescue, the consent of whom will only be provided upon a decisive vote by a quorum of the ManagTrustees Trustees. I understand that this agreement does not cover information which is already generally known or which I can show was known to me prior to becoming a Team Member.

I agree to always represent the Rescue in a positive and professional manner and to conform to policies and procedures as laid out by and made known to me by the Rescue. I accept that failure to do so may result in my role as a volunteer being ended.

I agree that if I leave the Rescue, I will remove myself from all and any social media groups including, but not exclusively, those that I have created on behalf of the Rescue and will provide full access rights to these to an existing member of the Team.

I agree to return any Property of the Rescue at my own cost if I leave, or am asked to leave.

I consent to these personal details being held on an electronic database and accept that they will be stored by PFBR for their use only and will never be sold or given to any other company or individual. I may ask to be removed from this database at any time. However, I accept that it will be unreasonable for all posts made by me, or including me, whilst part of the Phoenix Team, to be removed from any and all social media platforms.

I understand that by signing and submitting this form electronically I accept all terms and conditions of volunteering for Phoenix French Bulldog Rescue as agreed above.
